Indication
CoverGraf® is indicated for the prophylaxis of organ rejection in patients receiving allogeneic kidney, liver or heart transplants, in combination with other immunosuppressants.
Mechanism of action
Tacrolimus is a macrolide belonging to the class of calcineurin inhibitors (CNIs), which form the cornerstone of immunosuppressive therapy in solid-organ transplantation. It binds FKBP-12 to inhibit calcineurin phosphatase, blocking T-cell activation and the transcription of early cytokines (IL-2, IFN-γ) that drive the rejection cascade. Studies also support its use in glomerular diseases at the discretion of treating specialists.

Composition

For Healthcare Professionals
Kidney transplant: 0.1–0.2 mg/kg/day divided in two doses, every 12 hours. Liver transplant: 0.1–0.15 mg/kg/day. Heart transplant: 0.075 mg/kg/day. Dosage depends on concomitant immunosuppressants, corticosteroids and induction agents. Schematic monitoring of whole-blood trough levels is mandatory post-transplant; targets vary by post-transplant time-point and centre protocol.

Notes for patients & caregivers
Take at the same times every day, on an empty stomach (1 hour before or 2 hours after food) and at consistent intervals from any other medicines. Avoid grapefruit and grapefruit juice. Never skip or double-dose without speaking to your transplant team.
